To participate fully on CSDPP Assessment Tool, you must create a user account. -
Find Your Dashboard
To familiarize yourself with the Tool, review the Getting Started resources: The ASHP Guidelines on Preventing Diversion of Controlled Substances; Quick Start Guide; and FAQs. You may also download a PDF of the Assessment Questions so you can review or complete the assessment questions in advance of completing the online tool. -
Add a New Assessment
You can add as many assessments as you like by selecting the +New Assessment button. Typically, the assessment will equate to the organization in its entirety (e.g., a health system or hospital entity). You will then be asked to select whether the assessment is a Personal Assessment or an Official Assessment. You can create any name that you would like for each assessment. -
Select Your Assessment Strategy
If you select “Official Assessment,” the Tool will ask you to describe the scenario that applies to your organization and how you would like to implement the Assessment. This information is essential for preparing comparison reports. There are two options.- Option 1: Single Assessment. You must answer the Organizational-level Questions and complete the Action Plan. You can share the results with other team members if you want them to see the results. For example, this would be an option for a single entity, such as a hospital or free-standing surgery center where organization questions are applied only once or are the same across the organization.
- Option 2: Multiple Assessments. You can create multiple Assessments and assign them to other members of your team. The other members of your team will be responsible for completing the Organizational-level Questions and the Action Plan. They can add Sections and assign Sections to others on their team as they see fit. You have access to the results. For example, this would be an option for a multi-hospital health system where you would want each site diversion officer to complete the full Assessment for their individual hospital.

Answer Demographic and Organization-Level Questions
Answer each Organization-Level Question. The survey can be completed over several sessions; you can save your work and return to complete it later. Once you have completed all the questions then select “Next.” -
Add a Community Pharmacy, Pharmacy Location Only, or Work Site/Unit to
the Assessment
You can add an unlimited number of sections to an Assessment and have three choices, each with questions specific to that section: a community pharmacy; pharmacy location only (areas specific to the pharmacy operations); and work sites (locations where controlled substances are stored or used, but outside of the pharmacy, such as a nursing unit or surgery center). -
Inviting a Collaborator
A Collaborator is a person who was invited by the Account Owner to complete Sections of an Assessment for the Account Owner. Collaborators can only see questions within the section of the Assessment they were invited to complete. They cannot see the results or participate in the Action Plan. -
Completing an Assessment and Creating an Action Plan
Once you have completed all the Assessment questions, you can “Complete the Assessment.” Once the Assessment is complete, it cannot be changed, and you will then be able to create an Action Plan. You can also download your results in PDF format and your Action Plan as a PDF or CSV file. Tip: You must save the CSV file as an Excel or other file type or any changes you make may not be saved. -
Create, Download and Share Reports and the Action Plan
Once the Assessment is complete, you will be able to create reports and share results. -
Manage Your Assessment Details, Share Results, and Update Settings
You can manage your Assessments from the dashboard. Select “Details” to:
√ Manage (add or rescind) collaborators
√ Edit Assessment name
√ Change an Assessment from Personal to Official
Glossary
Community Pharmacy Questions - Community pharmacies are sometimes equated to retail pharmacies, operating out of both large and small chains or grocery stores. However, what constitutes a community-based pharmacy is much broader than the traditional retail setting. Community-based pharmacies also include outpatient pharmacies found within health systems, Federally Qualified Health Centers, primary care clinics, compounding pharmacies that prepare medications for patients who require unique dosing or modified formulations, and specialty pharmacies where patients receive outpatient care for complex medication therapies.
Official Assessment - An Assessment that accurately reflects an actual practice setting. Data from Completed Official Assessments are included in compiled data sets for benchmarking or other purposes. An Official Assessment cannot be converted to a Personal Assessment.
Organizational-Level Questions - Questions that address items from the Guidelines that are intended to be system-wide or organization-wide (if the organization is a single entity) policies and procedures that are universally applicable across the organization as related to the CSDPP.
Pharmacy Location Only Questions - Section of the Assessment that focuses on the pharmacy within the institution or health system. This section includes diversion prevention measures that are implemented within pharmacy locations and are unique to pharmacy such as central vault management, purchasing and receiving, and expired medications handling.
Personal Assessment - An Assessment that is just for the Account Owner’s private use. Data from Personal Assessments are not included in compiled data sets for benchmarking or other purposes. A Personal Assessment may be converted to an Official Assessment at a later time.
Work Site Questions - A section of the Assessment that focuses on a specific area within the institution or health system. There are 10 different types of work site categories: emergency department, nursing unit, etc.
